Centre back Alessandro Bastoni was unable to train with the team at the final training session ahead of the Euro 2024 last-16 clash with Switzerland in Berlin on Saturday.
The reason given by the national football federation was a slight fever for the 25-year-old.
Left back Federico Dimarco also trained with Bastoni away from the group
Bastoni missing the game would be a big blow to the holders, who are already without fellow defender Riccardo Calafiori through suspension.
Luciano Spalletti, who holds a news conference later, is likely to field 28-year-old Gianluca Mancini as Calafiori's replacement.
